Instructions

Trainee should report 10 minutes before the training hour.
Strict discipline will be maintained. The instruction of the Director or Coach shall be followed by the trainee, failing which they can be expelled without assigning any reason. The sole discretion will be of the coach.
The training school, kendra, academy, association or coach concerned will not be responsible in any way for any injury or accident sustained by the trainee during the training period.
Trainee must pay the fee by the 10th of every month, failing which a fine of Rs. 10/- shall be charged till the 30th or 31st. If a trainee takes leave for a long period, the fee will be charged for that period as well.
The name of the trainee shall be removed if the trainee does not pay the fee for 2 months, or is absent for more than 7 days without informing the Director or Coach.
Trainee shall be allowed to train in the prescribed uniform only.
Please also enclose a photograph and a medical certificate.
Grade promotion tests shall be conducted periodically. A successful trainee is awarded a certificate, and a meritorious player shall be eligible for a scholarship.

I accept the instructions and conditions above. I have read all of the instructions on this sheet and the declaration on sheet 1. I accept them as the conditions on which the player trains at Lucknow Taekwondo Academy, and I understand that the academy may end the player's training if they are not kept. A form returned without this box ticked and both signatures below will not be processed.
